Annapolis Valley Health provides palliative care in three “inpatient centres”, and in the community. They work with a number of community partners and multiple disciplines, to assure any resources necessary, are used throughout the patient’s palliative journey. They assess clients and existing supports and with the client's input, they determine their wishes, needs and concerns.
The palliative team is also available to support and educate health care workers and acts as an advocate for the “patient”.
