Single point of entry to this service. Inter-disciplinary consultation service. Consultation Team consists of a physician, nurses, social workers and volunteers. No in-patient unit rather a scattered bed approach throughout the three hospital sites which are: South Shore Regional Hospital – Bridgewater, Queens General Hospital – Liverpool & Fishermen’s Memorial Hospital - Lunenburg. Patients are seen in the clinic, home and hospital settings. Bereavement services are available. |
