The goal of this presentation is to describe a model of care that can inform long term care homes who are developing, delivering, or evaluating palliative care programs within their homes. The presentation will share outcomes that have emerged from research that has taken place in four long term care homes over a three year period. The presentation uses the CHPCA Square of Care and Organization as the norms of practice to define the scope and practice of quality palliative care delivery.
